2. No Provider-Patient Relationship

Use of this website, submission of a contact form, email communication, or viewing website content does not create a provider-patient relationship with Therapy & Mind or David Irias, PMHNP-C.

A provider-patient relationship is established only after completion of the appropriate intake process, informed consent documents, clinical review, and formal acceptance into care.

3. Services

Therapy & Mind may offer psychiatric evaluation, medication management, mental health consultation, trauma-informed care, EMDR-informed services or referrals, telehealth services, and appointment-based in-person services where available.

Services are subject to provider availability, clinical appropriateness, state licensure requirements, and completion of required intake documentation.

4. Emergencies

This website, email, online forms, and patient portal are not intended for emergencies.

If you are experiencing a medical, psychiatric, or safety emergency, call 911, go to the nearest emergency room, or contact a crisis line available in your area.

In the United States, you may call or text 988 to reach the Suicide & Crisis Lifeline.

5. Secure Patient Portal

Therapy & Mind may use a secure patient portal for intake forms, consent documents, appointment communication, billing, and treatment-related information.

Patients are encouraged to use the secure patient portal instead of standard email or general website forms when submitting sensitive personal, medical, psychiatric, or billing information.

6. Website Forms and Communication

Website contact forms and standard email may not be fully secure. Please avoid submitting sensitive health, psychiatric, financial, or emergency information through general website forms or standard email.

By contacting Therapy & Mind electronically, you acknowledge that electronic communication may carry privacy and security risks.

7. Appointment Requests

Submitting an appointment request does not guarantee acceptance into care or confirm an appointment. Appointments are confirmed only after scheduling, required documentation, and clinical review are completed.

8. Payments, Fees, and Billing

Fees for services may vary depending on the type, length, and complexity of the appointment. Payment policies, cancellation policies, and Good Faith Estimate information may be provided separately.

Therapy & Mind is now in-network with Cigna, Aetna, Anthem Blue Cross California, Carelon Behavioral Health, Quest Behavioral Health, and Regence BlueShield of Washington. If your plan is not listed, and you have a PPO with out-of-network benefits, a superbill can be provided for potential reimbursement.

9. Superbills

If superbills are provided, they are offered as a courtesy and do not guarantee reimbursement from an insurance company. Patients are responsible for contacting their insurance plan directly to verify out-of-network benefits, deductibles, coverage, and reimbursement eligibility.

10. Telehealth Terms

Telehealth services may only be provided when clinically appropriate and legally permitted based on the patient’s physical location at the time of service and the provider’s state licensure.

Patients may be asked to confirm their current physical location and emergency contact information during telehealth appointments.

11. In-Person Services

In-person services, when available, are provided by appointment only. Availability may be limited by location, scheduling, clinical appropriateness, and provider discretion.

12. No Guarantees

Therapy & Mind does not guarantee specific clinical outcomes, symptom relief, medication response, diagnosis, insurance reimbursement, or treatment results.

Mental health treatment is individualized and may involve benefits, risks, limitations, and alternatives.

13. External Links

This website may contain links to third-party websites or services. Therapy & Mind is not responsible for the content, privacy practices, policies, or accuracy of third-party websites.

14. Intellectual Property
